HomeMD Housecall Services currently provides primary care in over 100 independent and assisted living facilities throughout the state of Michigan from Lake Michigan to Lake Erie and everywhere in between. As a facility owner or manager your bottom line depends on keeping your rooms full, which itself is directly dependent on the quality of care you are able to provide to your existing residents. We understand that and we further understand that you need to be able to provide that level of care without exausting your staff through unnecessary phone calls coordinating things that should “just happen” without your need to intervene and follow up. At the end of the day it is your job to take care of your residents, not the medical professionals that are supposed to be helping you to accomplish your business goals.
Make your live easier, make your staff happier, and make your residents healthier . . . call HomeMD Housecall Services today to arrange for a time when one of our very own providers can come out to your facility to talk to you about how we can help your elder care business and the residents it is committed to providing care for.
As a housecall company we are often asked, “How can you help us to get more residents into our facility?” At HomeMD Housecall Services we like to reframe this question toward, “How can we keep your census higher by reducing resident turnover?”
How do we limit resident turnover? Through our quality health offerings we are able to reduce rehospitalizations and with our “always there for you” availability we are able to intervene in clinical situations faster so that urgent conditions do not turn into emergent conditions; in short, we keep your residents healthier and more functional for a longer period of time, extending the average resident lifespan and thus reducing your resident turnover.
Beyond reducing morbidity and mortality, you will find that our “turnkey” medical solutions for your facility will elevate your marketability. You’ll be able to tell family members of perspective residents that your facility is truly capable of providing an “aging in place” environment for their loved one while eliminating the need for the resident to have to venture outside the facility for routine medical care.
HomeMD Housecall Services takes a great deal of pride in our clinical availability, which allows us to market ourselves as the path of least resistance to our customers. When you call our office to report a clinical concern pertaining to a resident, you’ll be connected directly to your provider’s personal assistant. From there, he or she will handle your lower level issues on their own (ie. refills, records requests, etc) or they will contact the provider directly for higher level clinical issues, whom you will hear back from in minutes as opposed to hours or days.
After office hours and on weekends our lines are triaged by registered nurses who can field and provide solutions to most clinical concerns themselves, however, if the issue is something that requires a provider, we have on call providers available who will be happy to dive in . . . again, this process takes place in minutes, not hours or days, even outside of office hours. All facilities are provided with a tablet through which telephonic encounters can take place quickly and expediently over T-Mobile’s 5G network.
Your staff works hard to take care of your residents so it is important that you have a company equipped to effectively support your efforts. Stop spending your days chasing down outside medical providers, specialists, labs, diagnostics companies, and medical equipment companies and let us do all that work for you. Whether it be following up on a home health referral, ordering a walker, arranging to have medications delivered, or coordinating with family members, our office staff will happily handle all of those things on your behalf so that you can focus on what you do best . . . . taking care of your residents’ ADL and IADL needs.
To the right you will see a listing of the many services we can provide within our organization to your residents. We implore you to keep an eye on this list because it is growing daily. Whether it be a cardiology consult, mobile labs,/imaging, or even on-site dental care, we can arrange for these services to be provided in your facility through our company and because they are all provided under one roof, records will be available to your facility manager as well as to your resident family members through our EHR portal.
Through our electronic medical record, your facility manager will be empowered with easy, real-time access to all patient records 24/7/365 through our facility portal. No more making phone call after phone call to find out if labs were ordered or even what the results were . . . orders, labs, imaging results, advanced directives, DNR paperwork, provider progress notes, and anything else existing in our charts will be a mere mouse click away both to you and to the resident’s family members.
