Home-based primary care—or the modern-day “house call”—delivers primary care to the most medically complex (and costliest to the health care system) homebound patients in society. These patients typically have limited access to continuous follow-up care, which continues the cycle of poor health management and can become both expensive and exhaustive for patients and their caregivers.
In addition to enhancing the quality of care for patients and supporting caregivers, home-based primary care can significantly reduce medical emergencies, fragmented care, missed appointments, and the poor control of chronic conditions—the factors most frequently cited for emergency department visits, readmissions, acute hospitalizations, and institutionalization for this population, home-based primary care also helps alleviate stress and other challenges for everyone involved in the patient’s care.
At HomeMD Housecall Services we utilize the talents of our army of nurse practitioners who are uniquely equipped to handle the chronic and complex cases involved in geriatric care. As important as it is to have a grasp of medicine, we believe at our organization that it is equally important to have patience when dealing with the geriatric population. At HomeMD Housecall Services we believe “the 5 minute visit” is entirely inadequate for our geriatric population. While clinic based providers see an average of 20 patients per day, our providers see an average of 10 per day while boasting an average engagement time of 30-45 minutes with our patients and their family members during our encounters. When providing care to a population of patients who generally have multiple chronic conditions and who can sometimes take a little longer to articulate their symptoms, we find it impossible to provide quality care without taking the time to listen . . . this is the HomeMD difference!

Qualities of our home-based provider staff :
At HomeMD Housecall Services we employ staff that embody what we like to call the “Eight C’s” :
Providers should be comfortable caring for patients with complex medical conditions, who are often on numerous medications and have extensive medical records. The provider often needs to navigate complex psychosocial needs and challenging family dynamics. And, all of this must be done without the availability of specialists.
Caring for patients in the home involves not only caring for their multiple medical problems but also caring for the needs of others involved in the patient’s care. As such, it also requires a strong focus on goals of care and end-of-life conversations. Providers should be skillful communicators who are able to guide discussions on medical care and treatment with patients, caregivers, and other involved professionals.
Home-based primary care providers often work alone and need to demonstrate the highest integrity to ensure safe and quality care for the patient. Provider composure is essential because the medically and socially complex scenarios encountered at home, sometimes in neighborhoods with higher crime rates, may be challenging.
Home-based primary care providers are guests in the patient’s home, which is a very different dynamic than the office setting. Providers need to be highly considerate of this shift in “power” and of the patient’s personal and cultural wishes. Overall, just stepping into a patient’s home can be very informative and shows real compassion that can be reassuring to the patient and family in their difficult times.
